My system has /boot and grub installed on a USB flash drive. When I try
to upgrade the kernel, it fails; The boot partition is remounted as
read-only, and the device is reset, now using the ohci driver. If I
remount as read-write and attempt the upgrade again, the same thing
happens, except that the device disappears entirely. The only way I have
found to have the system recognize it again is to reboot.

I am able to reproduce this by copying a 10MB file to /boot, although
sometimes this needs multiple tries to trigger the issue.

Also, I do not believe that the drive itself is a problem. I was able to
reproduce the issue on a copy of the drive. I can also copy large files
to the drive using a different Ubuntu computer.

Please let me know if there's any other information I can provide.
