I seem to overlooked a detail above. >From Pacho Ramos partition list:
-- # fdisk -l omitting empty partition (5) <--- HERE (...) --- So even fdisk is complaning about error in the partitions. Combined with your example it seems to further confirm that an error in the partition tables causes this.
