Ok I figured out which add-on / extension was the problem with my bookmarks toolbar links disappearing. It was OpenBook 2.0.1.1.
Disabling that add-on, appears to have solved my problem totally, so I no longer have to run in safe mode. (Safe mode disabled all my extensions so I could not function that way for very long!) Thanks John, for suggesting that the problem could be related to extensions. When my bookmarks toolbar links disappeared, I was so scared. Losing my favorite places would have been so scary.
