I haven't yet figured out the cause of this. I'm seeing various other strange behavior also, such as nautilus windows needs to be refreshed manually in order to see changes.
On rare occasions all my menu entries are be duplicated, not the categories them selves but for example in Internet I have two entries for Evolution, two for Firefox and so on. Anyway I'm going to wipe this installation and install Feisty soon so I won't bother investigating this further.
