I have this same problem on my Thinkpad T43 running Lucid.

The problem happens intermittently for me, maybe once a day under normal
usage. Reloading the module fixes the issue.

The only interesting messages I can find in syslog are the following:

May 31 11:07:13 atomic-thinkpad kernel: [17876.016745] ipw2200: Firmware error 
detected.  Restarting.
May 31 11:07:13 atomic-thinkpad wpa_supplicant[851]: CTRL-EVENT-DISCONNECTED - 
Disconnect event - remove keys

I noticed that on my other laptop, using a broadcom chipset with the wl
driver, that WPA2 connections were quite unreliable and I frequently had
to do module reloads until recently. While I don't use unsecured
wireless that often, I can't say that I've ever noticed this problem
happen on non-WPA connections.

I am using a router with WPA2-AES authentication (not TKIP)
