Not that I have any idea if this is an issue or not, but on those
machines, was Windows shut down properly before you installed, or was
Windows Hibernated?

I'm not sure, in cases of hibernation in Windows, that the partition is
mountable, which may be an issue here.  I'll try to recreate this though
and see if I can get it to happen.

How exactly did you upgrade?  Did you do it via network (do-release-
upgrade or Update Manager) or did you do it with one of the ISO images
(which one?)
