I think this may be a pulse audio bug, try a workaround at https://wiki.ubuntu.com/DebuggingSoundProblems/KarmicCaveats. About half way down there is a section called "Volume range anomalies", if the information there sounds like what you are experiencing and you can fix it with the workaround they describe, i'd be pretty sure its PulseAudio. In that case please type "apport-collect -p PulseAudio" into a terminal to attach relevant debug information.
