My Kaffeine icon missing too. Package kde-icons-oxygen is installed but is not linked. I did it manually.
In Gnome from the menu System > Preferences > Main Menu > Applications > Sound and Video then select Kaffeine then select properties, then select the icon then browse to /usr/share/icons/oxygen/48x48/apps/ click open; then you should be able to scroll to the Kaffeine clipper board with a coffee bean.
