Is it possible that your usb stick hardware is physically damaged?
This could be due to unplugging it prematurely without unmounting it
first. This has happened to me and is not software related to usb-
creator, but an issue with education of the process necessary to safely
eject a usb disk device. If you find that you can no longer format this
one USB stick in any operating system, I think it is safe to say that
the hardware is damaged and that you should contact the manufacturer of
the removable storage device.
