Again, happens consistently on boot when running Nvidia 190.42 (or a 185 series) driver. I have now tried Nviida 195.22 beta driver and after multiple boots have never seen it occur. (Not saying that it is definitely an Nvidia driver issue...for example could be a race and using the newer driver may merely somehow change the boot timings...but does suggest 185/190 series Nvidia drivers as the problem.)
