Had similar problems on 32-bit version of Karmic. 
As advised in several places, get rid of Pulse-audio (sadly) en switch (back) 
to Alsa:

sudo apt-get remove pulseaudio
sudo apt-get install esound 
# be sure to check any muting of channels, can be done in graphic mixer, or 
commandline version: alsa-mixer

If this does not fix your problem, you can easily get pulse back using
the reverse commands.
