I had the same problem. I have exactly the same controller (AR5001X+ rev01) and it was working in Jaunty using madwifi (because the ath5k driver frequently froze my system). When I upgraded to Karmic today my card were no longer recognised.
I then tried to install madwifi-ng from source, following the instructions on this post (http://ubuntuforums.org/showthread.php?t=1309072). After that the card were recognised, but the speed was ridiculous ~30 kbps. Then I simply uninstalled madwifi-ng and removed the ath5k from the blacklist (/etc/modprobe.d/blacklist-ath_pci.conf). It's working perfectly now.
