By the way, in the situation described in comment #6 above, I did hear audio in the checkbox test where it plays a sound file to the default output--but only because I didn't follow the test directions and modified my default audio settings. To wit, I unchecked mute on output and increased volume level from <=11% to 20%. Filed bug 452037 concerning this issue.
