Just to add a "me too" and subscribe to this report:

I have the same prob on an Asus W2P laptop. Upgraded to 2.6.17-11 last
night and eth1 has left the building. lsmod does show ipw3945.

I also have access to a D-Link wifi usb "dongle" type device. If i boot
with that it is recognized as "wlan0". Note sure if this is important.
