I was able to easily reproduce...

If you're connected to a WEP AP, right click, select Edit Connections,
click the Wireless tab, and delete the AP in question.

Now go back and try to connect, but give it a bad key.  After it fails,
it'll show up with the "Never" tag.

I think a tooltip is the best solution as adding column headings might
clutter things up.
