My /proc/cmdline is
root=/dev/mapper/single-rootfs ro quiet splash

As you can see I have not booted with the "single" option but my lvm
volume name contains the word single.

This causes /etc/event.d/rc-default to put the system into single user
mode which brings up the recovery menu on every boot on Ubuntu Server.
