If you're using PulseAudio, please note that this symptom is exacerbated by bugs 315971/352732, which have been fixed in the latest pulseaudio upload (1:0.9.15-3ubuntu1): * debian/patches/0090_fix_sw_mute_desync.patch: Backport from git HEAD to resolve sw vol becoming muted on logout (LP: #315971, #352732) The changelog description doesn't mention the entire process of the hw/sw mute desyncing: on session restoration/login, pa notices that the sw vol is muted and attempts to mute the hw element (one of Master, PCM, or Front) to match.
