Could you try inserting the card while on VT-1 (hit ctrl-alt-f1) and see
if there is any output there on that terminal. You may find it helpful
to login on this console (using your normal username and password) and
run the command below, this will reduce the size of the font to allow
you to see more of the output:
setfont /usr/share/consolefonts/Uni1-VGA8.psf.gz
If anything is produced there it is worth taking a digital photo of this
output and attaching it here. It is also worth checking the syslog for
panic messages after resetting the machine following such a failure.
