I'm encountering similar issues on a Dell M1330 with a  4945AGN rev 61.
Upon resume from suspend, if the rfkill switch was on at time of resume,
to get wifi working again I must rmmod iwlagn then modprobe iwlagn after
re-enabling the wifi using the hardware kill switch.

I've attached my dmesg and  the output of `lspci -vnvn'. The most recent
resume had the problem.
