You should define HorizSync and VertRefresh for your Monitor, it seems the Frequency is too high (or too low). Maybe auto detection with DDC fails. If this doesn't help try the generator on this site http://www .bohne-lang.de/spec/linux/modeline/ to create a modeline at 60Hz, 70Hz, 75Hz (not higher). HTH
