It looks from the wvdial output like you're connected. If you are having trouble browsing, it could be that Network Manager isn't noticing the connection made by wvdial and is setting Firefox to "offline mode".
If this is the problem you are trying to describe, if you connect using wvdial as you have been doing, then open firefox and make sure that "Work Offline" on the File menu is *not* checked, you should be able to browse. The "DISABLED" ethernet interface is nothing to do with your wireless broadband interface, and can be ignored. That is related to your wired network port.
