I have a feeling this is related to opacity/transparence in 
gtk-window-decorator. I tried killing gtk-window-decorator, and restarting it 
manually with:
/usr/bin/gtk-window-decorator --active-opacity 100 --opacity 100

It seemed to help a bit, but not completely. However, the issues were
harder to repeat, and I never got the problem of the whole titlebar
disappearing, which could happen otherwise. Instead, only on a large,
maximized window did I get garbage across the titlebar when hovering
over the close and mimimize buttons.

I tried to install kde-window-decorator instead, but it failed to run
due to some unresolved dependencies. Whatever. I also tried "emerald",
which is another compiz window decorator. It worked, but it's not so
nice. Oh well.

So, for Jim and others: I have at least a workaround:
* sudo apt-get install emerald
* Go to System > Preferences > CompizConfig settings manager > Effects > Window 
decoration
* Change Command to "/usr/bin/emerald" (without quotes)
* Go to System > Preferences > Emerald Theme Manager to change the look of your 
window decorations. 
I chose: Edit Themes > Frame engine: legacy, and then on the topmost four 
(frame and titlebar), I set opacity to 1.0, and changed to color on active 
windows to something more Ubuntu-like.
