I don't have that system anymore :(

Maybe the problem is that initramfs-tools are installed after the
kernel. Maybe intrepid's kernel won't work with hardy's initramfs-tools.
In that situation, installation of new kernel wouldn't generate initrd,
but it would update grub menu.lst. And then, after initramfs-tools is
installed and initrd is created, grub never picks it up again. Thus, it
looks like everything is in place, but not in menu.lst. This, of course,
is just a wild guess. Too bad I don't have that system anymore, but I'll
try recreating this problem in VM.
