I'm seeing a similar problem when I attached my Sprint 3G phone by USB. The crash doesn't occur until I try to connect to the internet through the phone. The reason the applet doesn't come back is that it is the backend (NetworkManager) not nm-applet that crashes. Here's the backtrace. I have the full core-dump if that would be useful.
