Pardon for the long post, there was no attachment box on the initial report screen :-/
One thing I forgot to mention, hda3 and hda5 were partitioned using OSX's installer (Disk Utility), I selected "Unix filesystem" instead of HFS+ hoping that it meant ext2 or ext3 by "Unix filesystem". Wishful thinking, I suppose. But regardless, an odd partition type shouldn't cause the installer to crash. This is repeatable from my end, if you'd like to log and and see it for yourself let me know. Thanks once again!
