Thanks for the logs. The immediate cause (though not the bug itself) was
that the .disk directory is missing on your custom image; note this in
your original syslog:
Apr 5 14:53:57 custom ubiquity[7955]: Unable to determine the
distribution name from /cdrom/.disk/info
We fixed this a while ago:
ubiquity (1.8.4) hardy; urgency=low
[...]
* Fix crash if opening /cdrom/.disk/info fails (with Jonathan and Evan).
[...]
