Hast Du die preVars in der realurl.conf gesetzt? Bei mir sieht das 
ungefähr so aus:

'preVars' =>
    array (
                0 => array (
                        'GETvar' => 'L',
                        'valueMap' => array (
                                'de' => '0',
                                'en' => '1'
                        ),
                        'valueDefault' => 'de',
                        #'noMatch' => 'bypass',
                )
    ),
