Ich habe den Fehler selbst gefunden, es war ein falscher Eintrag in der .htaccess:
Nach Änderung von RewriteRule ^[^/]*\.html$ /index.php in RewriteRule .*\.html$ /index.php waren auch die Seiten ab der zweiten Menüebene erreichbar!
