AFAIK latest IIS and some Apache server support SNI, so I guess modern browsers support it as well. > Is it related to the HOST command? Basically I think it is, yes. However it is an TLS extension, the client just adds the DNS hostname to the SSL-Helo message during the SSL handshake process which enables server to present a matching certificate.
