Xorg server crashes on Dell Vostro V3750 notebook (integrated Intel GPU
+ Nvidia GeForce GT 525M) after upgrading xserver-xorg-core to
2:21.1.3-2ubuntu2.4.

After some testing we determined that the crash is caused by a patch re-
calculate-the-clock-and-refresh-rate.diff added in version
2:21.1.3-2ubuntu2.4 (LP: #1996490).
