Bug description:
I am trying to convert (parts of) a PDF document containing mixed
portrait and landscape pages to ps with fixed page sizes (so I can
feed it to psbook ... | psnup -2 ...), but pdftops can't handle that:
1. wget

2. evince DM00102166.pdf
Note that pages 44ff contain tables in landscape mode
3. pdftops -paper A4 DM00102166.pdf
4. evince DM00102166.ps
Note that pages 44ff are now empty, with:
(evince:6788): Gdk-CRITICAL **: gdk_pixbuf_get_from_surface: assertion
'width > 0 && height > 0' failed
