sound just disappears randomly when watching flash video in firefox

Bug description:
Binary package hint: pulseaudio
When watching a Flash Video (for example on YouTube) in Firefox, sound
just disappears systemwide.
Restarting Firefox does not fix the problem. I have to log out and
back in to my desktop session or restart my PC to get sound back.
I cannot reproduce the bug at will. It just randomly appears.
