"Focused" has confusingly varied meanings, but for a window the most common meaning is what in the Mir spec I call "input focus": receiving, by default, input from the keyboard and other non-pointing devices. Gimp's palettes, LibreOffice's tear-off toolbars, and Compiz's "Always on Top" function are examples of how a window can be in front even while unfocused. In the long run, for issues like pausing video or even suspending entire apps, whether a window is mostly/completely covered by other windows will be far more relevant than whether it is unfocused.
