Is it possible that the dialog is simply off-screen?

Try the following:

   - Invoke the dialog
   - Use the *keyboard* to invoke the window system menu (*ALT-SPACE*) *[N.B. 
   The menu may not be visible]*
   - With the *keyboard*, use the cursor down key (once) and then the 
   *ENTER* key to select the Move menu item *[N.B. The menu will likely 
   still not be visible]*
   - With the *keyboard*, use the cursor right key (once) to move the 
   window a little to the right *[A side-effect of this is that the mouse 
   is now attached to the window]*
   - Now move the *mouse*

If the window was off-screen, it should now be visible.

If this isn't clear, first try the process on a window you can see (e.g. 
start Notepad). Having proven the technique, try it on the dialog you are 
having problems with.
