That is no longer true for US hams, since the 1970s. Our license is an operator license only, the address is a mailing address where we receive official communications from the FCC. :) My callsign is simply K9YC anywhere within the US. I live in California.
Before that time, we had a single piece of paper with two licenses -- one for operator privileges, the second for the station at a single location, and we had to sign /n at any other location.
