When the defaultHost attribute on Engine is not correctly pointing to a Host
element's name, there are no errors generated and no error documents are returned
to the web browser.  This can cause, in my case, many hours of "debugging" until
you realize this.  In fact, no log entries are generated for access, etc.

This is a big "useability" issue for setting up new servers.
