[toaster] qmail-inject error, mail dropped with no error to sender

Hi,

any ideas is appreciated
I get some mails that are dropped by toaster:

/did_0+0+1/

First - why does it care about the "to:" line, not the envelope rcpt?

Second - if there is a fatal qmail-inject error, why is this considered 
 as "success" (and therefore no error is returned to sender)
