Try Qscanq and clamav.
Anyone using this with SpamAssassin? I've got a RH ES box with the toaster and it has the exact same issue. This does not seem to be a qmail-scanner thing though as the mail is being resent from the originating server, which means our qmail boxes are not saying "message received" correctly causing a resend, it also doesn't seem to affect all sending servers as I've only got this issues with a few so far.
I use qscanq/clamav with spamassassin, but spamassassin is on a per-user config, not site-wide. Works great.
