The database from http://tw5magick.tiddlyspot.com/ is very old and make use of a button, that would not work now. Have a look at a solution also older and also from Stephan Hradek here <https://groups.google.com/g/tiddlywiki/c/GHrWuZAQIN8/m/voZqHiG04DUJ> I think it would be far easier for you to change for your purpose.
I remember at that time we had some problems copying the code - linebreaks and more. I do not want to offend anyone, but to avoid problems for you I will upload the tiddler: Fill out the form and see what happens, when the buttons show. Keep an eye on what happens in sidebar in tab recent also. It is very easy when you get used to it.
