For windows, SecureCRT is a good one - but it is shareware and will expire after 30 
days.  It's pretty easy to find on download.com or tucows.com, I'm sure.

I prefer TerraTerm Pro with the ssh plug-in, I use it at work a lot and it has worked 
flawlessly - and the ansi emulation is great.  The source code is also available on 
the website:   http://hp.vector.co.jp/authors/VA002416/teraterm.html

I'm actually looking for a good ssh-server how-to also, so I'll let someone else 
answer that part of the question.
