You can do this with two ways.
1. Do an Talks followed by a long Up Arrow to spell the item in question.
This is spell current line.
2. Try doing a Talks followed by a long Right arrow. This is spell current
word.

In the Talks settings, you can set your spelling speed to a slower speed if
you need.

If you change cursor mode with talks long key 2 from  original to letters
you should be able to use the arrow keys to read letter by letter.
