if you want to add so much information, I think it would make sense to create an amenity=toilets object instead of sticking it on the POI, this will give information on its position and will also allow to deal with the case of 2 toilets not having the same characteristics

toilets:wheelchair_door_width                (for the door width of the toilet)

the correct namespace separator is ":", no the "_"
so if you want to glue all info on the poi
toilets:wheelchair:door:width
or an amenity=toilets + wheelchair=yes + door:width=*

toilets:wheelchair_space_side                   (for the space next to the toilet)

toilets:wheelchair:space_side

toilets:wheelchair_space_front                 (for the space in front of the toilet)

toilets:wheelchair:space_front

shower:wheelchair                                       (for a wheelchair accessible shower, answer with yes/no)

or it's own object with amenity=shower + wheelchair=*

With the On wheels app we measure the narrowest point from the entrance to the toilet. We want to give this an OSM tag, but it is hard to find a good one. There are no indoor tags that describe a narrowing of a path. A possibility is to add the > following combination of tags: highway=path, indoor=yes, width=.

that's fine

For the moment our app has no option to add separate
indoor tags like toilet, elevator, doors, … So all the tags are connected to the main tag of the building.

this will severely limit you unless you introduce some very twisted rules.
a poi for example has 2 amenity=toilets, which do not have the same characteristics in terms of space, door width, etc
How are you going to fill this in on a single object?
