I'm mapping them as highway=stop as a node of the way, just before where it crosses the main road. I would map a traffic_sign=stop as a node next to the road. But that just makes it harder to work with the data, so I prefer the first approach.
What I never do anymore is to tag the node of the crossing with it. So even if all 4 roads have a stop sign, I'd create for nodes for them on all approaches.
