currently I'm mapping them with barrier=wall (U-form) and
highway=steps in the middle (and on the end subway_entrance) but it
would be nice to have them show up.

you could use barrier=entrace, but this is according to my reading of
the wiki a hole in the barrier without door or gate, that's why I
usually use barrier=gate for general entrances (which do have a
door/gate).

name? proximity? That would get some false positives (double-stations)

well, but name is for the name of the feature, so it could well happen
that a subway entrance has it's own name that is not the one of the
station but the one of the street it is leading to.
I'd opt for the relation.
