Shared knowledge, FIO. UBS (United Bible Society) has some proprietary software called http://confluence.ubs-icap.org/display/PUBASSIST/Home Publishing Assistant that takes the USFM from Paratext and puts it in a format that interfaces with http://en.wikipedia.org/wiki/Adobe_InDesign InDesign .
If they set all the parameters well, then there is only tweeking to make the columns line up nicely etc but, still a lot of work. In the end they get a very nice looking Bible.
