I had the same issue (also solved it by moving the stuff myself) but thought it might be limited to my rather non-standard setup (I switched my profile back and forth between en-US and de-DE in the past and also applied lots of customizations over time). If this is indeed more common we should at least verify under which circumstances it happens and whether the same applies to upgrading Firefox from a pre-Places version to, say, FF5. Then either file/fix a bug or at least add it to the Known Issues / FAQ.
After doing this, I exported the bookmarks to bookmarks.html and looked at that in the browser. I see that what populates the toolbar is now the "Bookmarks Toolbar".
Good to know.
I suggest that if a bookmarks.html file is imported, that the "Personal Toolbar" should not be ignored.
