Kamailio comes with some example configs (atleast the debian packages), both
kamailio-basic.cfg and kamailio-advanced.cfg contain the examples for mysql
and rtpproxy. All you have to do for rtpproxy for all calls is to move the
rtpproxy_manage("co") before the NAT checks.
