Its indeed outdated, and probably should be removed.
a command for upgrading the structure of database should be easy to add,
at least for mysql, the full sql script is at:

http://www.kamailio.org/wiki/install/upgrade/3.2.x-to-3.3.0?&#sql_commands
However, updating existing records might require more than update of
table structures.
