Try enable mhomed=1 option in kamailio.cfg.

If this does not do what you want, explicitly force the outgoing interface by manipulating the $fs pseudovariable prior to relaying/forwarding, e.g.

   $fs = "udp:xxx.xxx.xxx.xxx:5060";

   t_relay(); ...
   etc.
