Are you very sure this connection wasn't whitelisted or authenticated? I'm only asking because every other report about problems with the header blacklist filter has ended up being a whitelist issue. If you're running the latest version of spamdyke, the ALLOWED entry in the mail log should show a whitelist or authentication.
If that's not it, I'd suggest searching the mail logs for any rejections from the blacklist filter -- is it working at all? If not, this could be a problem with your configuration file or "run" script. If none of that is true, it may be a bug. Would you mind recompiling spamdyke with "excessive output", turning on full logging with the "full-log-dir" option and sending me the log file from one of these messages?
